Course Evaluation Checklist 

All these materials can be picked up in office N2305 (Academic Affairs) and returned to the plastic bin in her office when completed.

•  RED forms for each student in the class (stack divided into groupings of 25)
• One GREEN form to be filled by faculty
• One YELLOW sheet for instructions
• One BLUE sheet if Blended Learning course
• One PINK half-sheet if Ed. Dept.
• One SMALL SLIP with Supplementary Q’s
• PENCILS for each student (bundled in 25s)
• One MANILA ENVELOPE to return forms to Academic Affairs
