  1. Upload insurance information go to the “Insurance Tab” and then click “Add New Insurance”
  2. Enter all the information for your insurance plan

  3. Lastly make sure to upload pictures of your insurance card. Be sure to follow the instructions given by ATS. Either do Option 1 or Option 2. NOT BOTH
