  1. After logging in, select Athlete Information
  2. Once you get to this screen you will go to the “Forms” Tab
  3. “Forms” Screen will appear like this and then to start forms click the drop down located next to “Form Name”

  4. If you are an incoming student (Freshman or Transfer) a physical must be done. You will need to download that form under “eFiles” as well as submit/upload under “eFiles” once complete
